Job Description

Chestnut Health Systems is a leading provider of behavioral and primary health care services. We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers. Responsibilities

  • Provide individual, family and group counseling to clients and families involved in our adult substance use programs according to the CRA and CRAFT model of care.
  • Conduct diagnostic and assessment evaluations and place clients in the appropriate level of care.
  • Develop Master Treatment Plans and complete continuing care plans with clients.
  • Provide regular presentations to family members on behavioral health information.
  • Demonstrate flexibility in schedule so as to meet families when they are available, including some possible evening hours.
  • Maintain records of client and all services provided within assigned program in a timely fashion and according to policies.
  • Reflect a client-centered, strength-based approach to clinical work with family.
  • Attend court meetings as needed for Drug Court services.
  • Attend and participate in all program and staff meetings as assigned.
  • Participate in program and staff development activities.
  • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
  • Perform other duties as assigned.

Qualifications

  • Must have a master’s degree in a human services field and obtain certification as a Certified Alcohol and Drug Abuse Counselor (CADC) or obtain licensure as an LCPC, LCSW or LMFT within two (2) years’ of hire.
  • Must be certified in Community Reinforcement Approach (CRA) and Community Reinforcement Approach Family Training (CRAFT) within two (2) years’ following CRA training.
  • Experience working with families preferred.
  • Must have a valid driver’s license, private automobile insurance and be insurable.
  • Requires basic computer skills and experience to include Word, Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
  • Satisfactory completion of Chestnut’s background checking process.
